"An investigation involving multiple systematic reviews and a consensus study of the effects and definition of squatting position in childbirth"

Abstract:
The primary purpose of this Doctoral research programme is to establish an evidence-based understanding of the squatting position in childbirth. The aim to achieve this is through a systematic approach that enhances the design and reporting of future randomised controlled trials. The overarching objective of the thesis is to compile a comprehensive body of evidence on the squatting position in childbirth by conducting multiple secondary analyses of existing studies and conducting primary research using consensus development methods. The thesis is composed of eight chapters. Chapter 1 introduces the field of analysis, defines the research topic's context, and outlines specific aims, objectives, and the structure of the thesis. Chapter 2 conducted an overview of Cochrane systematic reviews as a foundation to generate hypotheses for subsequent chapters. Chapter 3 focuses on biomechanical characteristics and consequences of the squatting position in childbirth. Chapter 4 investigates the effectiveness of the squatting position compared to other positions during childbirth, using RCTs and non-randomised studies of interventions and examining maternal and neonatal birth outcomes. Chapter 5 explores the factors that influence the use of the squatting position in childbirth. Chapter 6 analyses pre-specified and post hoc subgroup data derived from RCTs. Chapter 7 describes a Delphi e-survey and nominal group technique discussions to establish a consensus and expert opinion on the precise definition of the squatting position. Chapter 8 serves as the discussion and conclusion chapter, summarizing the evidence-based framework on the squatting position in childbirth and highlighting its implications for clinical practice and future research.
