Combined medical therapy and carotid endarterectomy for asymptomatic stenosis: 10-year stroke prevention in ACST-1
- Abstract:
- In a randomised trial (ACST-1) of 3120 asymptomatic patients withsevere carotid narrowing comparing early with deferred carotid endarterectomy(CEA), CEA significantly reduced 10-year stroke risk. All other aspects ofpatient treatment were left to the discretion of the clinician. Throughout thetrial antithrombotic and antihypertensive drugs were widely used but use oflipid-lowering therapy increased substantially. We examined the influence ofdifferent medical treatments on the overall stroke prevention effect of the trial.
